Exosomes are the treatment the aesthetics industry has spent the last two years talking about. They are tiny vesicles that cells use to signal to one another, and in aesthetic medicine they are used to encourage the skin or the scalp to behave a little more like it used to. The interest is genuine. So is the marketing, and the two are not always easy to tell apart.

This is an area where the gap between clinics is unusually wide. The product a clinic uses, where it comes from, how it is delivered and who assesses you beforehand vary enormously, and the treatment is early enough in its life that honest clinics are still careful about what they promise. That is the spirit this guide is written in.

A note on transparency: this guide is published by Trikwan Aesthetics, and we have placed our own clinic first. Our criteria are set out in full below so you can judge that for yourself. The other four clinics are genuine, well-regarded providers, each strong in a different way.

What You Should Know Before You Book

One point matters more than any other, and it is the one least often volunteered. In the UK, exosome products are not licensed as injectable medicines. Most careful clinics therefore apply them topically, immediately after microneedling or a laser pass has created channels in the skin, rather than injecting them. If a clinic offers to inject exosomes into you, that is a fair moment to ask exactly what the product is, where it is made and on what basis it is being used.

The second point is expectation. The published evidence for exosomes is promising and still developing, which is not the same as proven, and any clinic telling you otherwise is ahead of the science. How We Chose These Clinics

  • Transparency about the product. The clinic can tell you which exosome system it uses, who manufactures it and what data sits behind it, without being asked twice.
  • How it is delivered. Topical application after microneedling or laser, injection, or a patient-derived route. Each is a different proposition, and the clinic should explain which it uses and why.
  • Who assesses you first. Hair loss and skin change both have causes worth diagnosing before anything is applied. A clinic that goes straight to the treatment has skipped a step.
  • Honest framing. A course, a realistic timeframe, and a willingness to say the evidence is still developing.
  • What it is combined with. Exosomes are usually part of a plan. The plan should be explained to you as a whole, including what it costs you in time.

The advantage of a dermatology-led route is diagnostic. Hair thinning and skin change are exactly the presentations where an underlying condition can be missed by a purely aesthetic assessment, and a dermatologist is the person best placed to catch it. If your concern is scarring, pigmentation or scalp inflammation rather than general skin quality, start here.

3. PRP London Clinic (Best for Choice of System)

At 33 Cavendish Square, PRP London Clinic delivers exosome protocols through GMC-registered doctors, and its distinguishing feature is that it does not commit to a single product. Alongside a manufactured exosome system, it offers a route that generates exosomes from your own cells, which sidesteps a good deal of the debate about sourcing. Being offered a genuine choice, with the reasoning explained, is a good sign in a field where most clinics carry one product because that is the one they were sold.

4. Continental Skin Clinic (Best for Needle-Free Scalp Treatment)

Continental Skin Clinic in Kings Cross takes a different technical approach to the scalp, using laser-assisted delivery rather than needling to get the product where it needs to be, paired with LED therapy. For anyone who finds scalp needling uncomfortable, that is a meaningful difference rather than a marketing one. The clinic is also unusually clear about why it chose its particular exosome system, citing the manufacturer’s published data, and it is careful to describe some of the more eye-catching claimed benefits as supportive rather than expected. That restraint is worth something in this category.

5. The Axis Clinic (Best for Doctor-Led Care Outside the W1 Postcodes)

The Axis Clinic in Crouch End offers exosome treatment for both skin and hair, delivered by Dr Carlos Escobar, whose background is in neurosurgery. It is the entry on this list furthest from the Harley Street cluster, and it is here because doctor-led regenerative treatment should not require a trip into W1. For North London patients who want a medically qualified assessment without the central London premium, it is a straightforward recommendation.

Which Clinic Is Right for You?

If your concern is hair, get the cause diagnosed before you pay for anything, which points to a dermatology-led assessment first. If you dislike needles, the laser-assisted route is worth the journey to Kings Cross. If you want to weigh a manufactured product against one made from your own cells, see a clinic that offers both. And if what you actually want is better skin quality overall rather than this specific treatment, find a clinic that will happily talk you out of exosomes and into something more established.
