The AAMG: Keeping Trikwan Aesthetics at the Forefront of Aesthetics Research

Trikwan Aesthetics’ medical directors, our husband and wife doctor duo, Zoya Diwan and Sanjay Trikha, have based their practice on the latest research and evidence-based treatments for their loyal clientele. However, they wanted to go further in their contribution to the aesthetics profession and to aesthetics research, and so in 2018 they founded the Academic Aesthetics Mastermind Group (AAMG), with the aim of advancing the field of Aesthetic Medicine through education, research and collaboration with their fellow aesthetics practitioners. As part of this mission the AAMG, led by Dr Diwan and Dr Trikha, are carrying out research into all aspects of aesthetic medicine and treatment in order to achieve the group’s aims: furthering aesthetics education and research; building a community of likeminded research-driven practitioners; and, importantly, improving treatment outcomes for our patients.

The AAMG’s latest paper is our first to be published in the prestigious journal of Plastic and Reconstructive Surgery and was a collaboration between 6 of the aesthetic practitioners that help found the group, including Dr Trikha and Dr Diwan. It details the experiences and findings of 4 of these doctors in performing Tear Trough fillers and evaluated the safety of their cannula technique, the same that we use here at Trikwan, in carrying out this treatment.

Our paper assessed the benefits of using the relatively new cannula technique for Tear Trough filler placement. This technique uses a blunt-ended tube (a cannula) rather than a sharp needle to place dermal filler under the eye to rejuvenate the skin and add volume to this area. Because it avoids the use of sharp needles that could damage the delicate structures and blood vessels around the eye, the observations of our paper found that this technique is much safer, and produces far fewer complications, in comparison with reported treatment outcomes using the traditional needle method. While prolonged bruising and swelling has been a common complication resulting from the traditional needle method, the cannula method that our Trikwan doctors use, saw very few patients who suffered these complications and none for which any bruising or swelling didn’t completely resolve within 1 month.

The AAMG’s collaborating doctors also found that, when using this technique, our patients reported a very low level of pain during treatment and a high satisfaction rate of 75% 1 month after treatment, with the main reason for patient dissatisfaction being financial, in that they may be unable to afford the cost of the extra filler needed to completely resolve their tear trough deformity. Despite this, all of the clients treated by our AAMG doctors using this safer technique, found that there was a definite overall improvement in the appearance of their under-eye.

Our AAMG paper has also helped to inform aestheticians about the best advice and aftercare for our clients undergoing Tear Trough treatments. We found that our patients over the age of 35 are more likely to require more than 1ml of dermal filler between both under-eyes to achieve their desired result, and so, as good practice, we and other aesthetic practitioners should advise our patients of this during consultation. We also found that the best time to invite our patients for review of their tear troughs is 4 weeks post-treatment, and that if more filler is desired, that this is the earliest that any further treatment should be performed.

The findings from our first PRS journal paper is helping to advise our treatments here at Trikwan, alongside our fellow UK and internationally based aesthetic practitioners, in the treatment of the under eye and tear trough with dermal fillers. It aids us in providing the safest treatments and high-quality aesthetic results, ultimately improving the experience for all of our patients.
